Explanation of fees:

    First visits are scheduled for
    approximately 60 minutes, and
    follow-up appointments generally last
    15 - 45 minutes. Since the follow-up
    visit is crucial for optimal treatment
    and healing, it is included in the price
    of the initial appointment. Follow-up
acupuncture visits are scheduled for approximately
45 minutes.


Fees are as follows:

Initial visit and first Follow-up visit                        $365.
              (depending on complexity)

Follow-up appointments are generally:                $ 125.
but may range from:                                     $ 65 - 155.


Acupuncture                                                        $ 105.


Telephone calls under 5 minutes:                          $   0.

Telephone calls over 5 minutes will be treated
and billed as an office visit



The pricing structure is subject to change, and we will try
to inform patients of any changes prior to scheduling
appointments.

Payment is expected in full at the time of the visit unless
other arrangements have been made. Dr. David accepts
credit card, check and cash for office visits and
supplements. A 10% discount is offered on supplements
only if paid by check or cash.

At this time, we are not accepting insurance. However, we
will gladly provide you with a fee ticket containing
insurance billing codes for you to submit to your provider,
for out-of-plan health care reimbursement.
